Dear McCallum High School Families,
I hope y'all are having a great summer!
In anticipation of next school year, I am attaching the forms for Individual Health Care Plans for diabetes, asthma, seizures, and allergies, as well as a blank form for any other illnesses or diseases that you feel that I need to be aware of for the school year. If your child has a condition that could require emergent intervention by the health clinic staff while at school, please have your child's healthcare provider fill out the appropriate form and submit it, along with any needed medication, during the start of the school year. The more information I have, the better prepared I can be to look after your kids!
I am also attaching the medication administration form. ALL medications (including over the counter medications like Advil, Tylenol, Tums, and Benadryl) need a doctor's signed order in order for our Health Clinic staff to administer. If your child deals with cramps, headache, nausea, etc., help us help them by having this form filled out and a new, unopened bottle of medication on hand at school.
Also, please take the time to make sure your kiddos are up-to-date on their vaccinations over the summer! For those with exemptions, this is a good time to check to make sure your exemption is not expired. They expire every two years!
