Pre-Participation Physical Forms for the 2024-2025 school year are now available
Free Physicals will take place May 7th and May 9th
Pre-participation physical forms for the 2024-2025 school year are now available. All students participating in athletics (MS/HS), cheerleading (MS/HS), marching band (HS), 8th grade band students enrolling in HS band next year (MS), drill team (MS/HS), or dance team (MS/HS), will be required to obtain a new physical. This physical must be dated after April 15th, 2024 and will be valid for one school year. The completed form is to be returned to the coach or sponsor prior to participation in the new school year.
Physical forms may be found on the AISD website, in campus athletic offices, and at the AISD Performing Arts Center.
For students without a primary care physician or the ability to obtain a physical, the district is partnering with local physicians to offer free physicals. These physicals will take place on May 7th and 9th. Registration is required to attend.
To receive a free physical, students must turn in hard copies of the required paperwork to their Coach/Director in advance. Coaches/Directors must give completed paperwork to Denise Vanlandingham at Burger Center by April 30, 2024.
