• Child's birth certificate
• Child's Immunizations
• Enrolling Parent ID
• Enrolling Parent Proof of Residency

Friendly Reminders:

• Proof of Residency must be from the last 30 days.
• Must be a utility bill (gas,electric, or water), lease contract, or mortgage statement
• Enrolling Parent must be present.
